By Enrollment Form

Many School Groups wish to purchase their insurance coverage and mail in a check. No problem! Download Enrollment Form (pdf) complete it and mail it in. It’s that simple.

You can add easily add Additional Insureds and print unlimited Certificates of Insurance by Signing in right here on our website.

Have coverage changes? Or other changes to your policy? Please email requested changes to support@rvnuccio.com. We process the change immediately and send an email with a change endorsement and correct documents.

Certificates of Liability can be added during the purchasing process online. You can also add additional Certificates of Liability to you policy after purchase by signing in with your username and password.  Once signed in, select the application and click “Add Additional Insured/Certificate of Liability.” Add the Additional Insured information and click “Finish,” to print the endorsement and Certificate of Liability Insurance.

Does your school district require you to add them to your policy as an additional insured? You can add an Additional Insured when purchasing your policy. You can also add additional Certificates of Liability to you policy after purchase by signing in with your username and password.  Once signed in, select the application and click “Add Additional Insured/Certificate of Liability.” Add the Additional Insured information and click “Finish,” to print the endorsement and Certificate of Liability Insurance.

Policy documents are available to download immediately after purchasing. In addition, the Booster Club Policy documents are emailed as PDFs. To reprint policy documents, sign in to view and/or print your policy documents. Need assistance? Call us at 1 (800) 567-2685, and our customer care representatives will assist you.

To report a claim, please call our office at 1-800-567-2685 between the hours of 5:30 AM and 5 PM Pacific Time. A representative will assist you with filing your claim.

All of our Booster Club policies are annual policies and are not available on a short term basis. Our One Day Special Event policies are available on a short term basis.
